You just simply need to add a tag in the product tag section of a certain product in here:
Just simply add a product tag: personalisation-YOURTEXTHERE
personalisation-child's_age-2

personalisation -> the tag needed for it to display.
child's_age -> The Label that would appear in Product Page (Please use "_" for spacing)
2 -> The Max Limit Characters

PS: If you're looking for Image/Video Upload, you would be needing an app for this to achieve, as this is something only a third party app can do.
